  • Decree Absolute if one of the applicants is divorced
  • Death Certificate of the spouse if the applicant is a widow
  • Letter of Confirmation of Marital Status from the country of residence (Foreigner)
  • Application Form JPN.KC02
  • Two (2) witnesses to bring along Identification Card (Malaysian) / Passport (Foreigner)
